Error Loading CSV File

I'm loading a CSV but I've got an error below. Attached is the sample CSV.

The original CSV was generated in MT5. I've uploaded a CSV generated from MT4 which was successfully imported. When I copied the same line on 2014.11-20, I didn't see any difference in formatting except for the difference in the values.

Even I selected the MetaTrader 5 or custom file format, I still have the same error.
