Missing template for ATR Volatility Sizing (v.137)

when I switch Money Management to "ATR Volatility Sizing" I got error that template is missing, see attachments.
SQX v.137.1749

Hello, I assume that you have downloaded it from our site

It was used as an example how to add a new MM method to SQX.

If you want to use that MM in MT4 you have to implement the missing template file.

but there missing some guide, how we can create missing template file, or please some link to "How to"
